About the Role

The Member Wellness Coach is a hands-on, onsite leader responsible for advancing members’ cognitive vitality, mobility confidence, emotional well-being, and social engagement.

This is not a desk-based position. You will be actively engaged with members throughout the day — observing patterns, reinforcing healthy routines, collaborating with program staff, and ensuring each member feels seen, supported, and valued.

The Member Wellness Coach plays a central role in translating longevity science into practical, daily experiences that help older adults thrive at home.

Key Responsibilities

Member Wellness & Daily Engagement

  • Conduct structured wellness check-ins aligned with cognitive, physical, emotional, and social domains
  • Observe and document changes in engagement, mood, mobility, or daily rhythm
  • Reinforce healthy routines that support aging in place
  • Provide active supervision and engagement on the floor during programming hours
  • Collaborate with the Social Engagement Manager to integrate wellness themes into daily activities
  • Support members during transitions, group sessions, and structured engagement blocks , including ADL/IADL assistance as needed

Family Communication & Relational Support

  • Serve as a trusted point of contact for members and families
  • Conduct proactive outreach when engagement patterns shift
  • Support families in understanding evolving needs while reinforcing independence and dignity
  • Maintain clear boundaries between lifestyle wellness services and clinical care

Programming & Wellness Integration

  • Align programming with cognitive stimulation, mobility preservation, emotional resilience, and social connection
  • Co-lead educational sessions related to brain health, fall prevention, stress resilience, and healthy routines
  • Support dementia-informed engagement strategies
  • Contribute to participation tracking and engagement insights

Safety, Documentation & Compliance

  • Maintain accurate documentation of member engagement and observations
  • Support adherence to required staffing ratios and supervision standards
  • Participate in emergency preparedness and response
  • Maintain CPR/First Aid certification
  • Uphold confidentiality and site compliance standards

Clinical Partner Collaboration (If Applicable)

At select locations, the Member Wellness Coach may collaborate with external clinical partners under appropriate supervision to support care coordination initiatives. Responsibilities may include:

  • Supporting member participation in structured wellness outreach
  • Reinforcing provider-directed goals
  • Assisting with communication following hospital or rehabilitation transitions

The Member Wellness Coach does not diagnose, prescribe, or provide medical treatment.

Qualifications

  • Bachelor’s degree in Gerontology, Public Health, Social Work, Psychology, Kinesiology, Recreational Therapy, or related field
  • 2–4 years of experience in adult day services, senior wellness programming, community health, or similar environment
  • Ability to read, write, and speak English. Bi-lingual encouraged to apply
  • Strong observational and communication skills
  • Ability to build trusting relationships with older adults and families
  • Comfort working in a dynamic, hands-on, community-centered environment
  • Strong organizational and documentation skills
  • CPR/First Aid certification (or willingness to obtain)
  • Level 2 Background check and TB screening required
