Overview
Social Worker – Medical Social Worker (Travel) for a short-term assignment in Portland, Oregon. Start date: 04/13/2026. Assignment length: 13 weeks. Weekly pay: $1,865–$2,025 per week. Shift: Day shift, 8 hours, 5 days per week (0800–1630). Setting: Short Term Acute Care Hospital with an ambulatory/primary care focus; onsite, with responsibilities across clinics as needed. This travel position is a temporary assignment through Healthforce Healthcare LLC.

Responsibilities
- Provide medical social work services in an ambulatory/primary care setting, including psychosocial assessments and care planning
- Facilitate discharge planning, patient education, and linkage to community resources
- Conduct counseling and crisis intervention as part of interdisciplinary care
- Document assessments, plans, and outcomes accurately in the charting system
- Coordinate care with the multidisciplinary team and support remote clinic operations as needed
- Assist with transition of care and follow-up to ensure patient and family needs are addressed

Requirements
- Education: Bachelor’s degree and Master’s degree required
- Certifications/Licensure: LCSW preferred; CSWA preferred
- Experience: 3 years of clinical/social work strongly preferred; hospital clinical experience is a plus
- Licensure notes: Licensure in good standing preferred (LCSW); CSWA also considered
- Work authorization: Ability to work onsite in Portland, Oregon; valid in the US
